do you mean who is responsible for their welfare? or who is responsible for their state of being/condition???

It's their own fault they ended up the way they did. They can get help if they want it. Most of them don't. Leave them be. If they wake up and decide to take care of themselves great. The vast majority of them won't. Don't waste your energy trying to help people who won't do anything for themselves.
