umm number one you just let the free world into your personal life with all that information,,, a big no no! to answer your question look for the company phone number online or in yellow pages call them they can help you, and for future references no personal info aka account # and home addresses online it will save you alot of headaches in the future

NEVER GIVE OUT YOUR ADDRESS OUT ON THE INTERNET!
What internet service provider do you have or phone company.
Contact them directly or visit their website for help. You might be able to pay it online or through your banks bill pay.

I agree with everyone, don't post all you personal info. on the web. So not smart! I don't even understand why you need to add all that info. to your question.
