I used to work the 911 hot-line and I received a call from a guy who put superglue in his eye... I don't know why or what he thought he was putting in his eye.
My best friend went to put eye drops in her eyes, but used the drops for the dog. She could not see for a week. Her eyes were so dilated that she had no color in her eyes... our boss thought she was doing drugs.
I personally made the mistake of using prell shampoo after spending hours coloring my hair (it strips out the color).
Oh yeah, and going to the bathroom right after putting bengay on my knees. OUCH!!!!

This was very embarrassing at the time it happened. When I was a little kid probably 9 or 10 years old I used conditioner instead of shampoo to wash my hair. My mom questioned me wether I washed my hair or not, by this time we were at the mall and my hair was embarrassingly dirty the remainder of the day.
I also had a few baking misshaps which I need not elaborate on...

Mine is the inverse of a lot of the answerers' above mistakes. I once put a packet of sugar on an order of fries instead of salt. It was not good.

Well, once when I was little (about 7), I wanted to make my parents breakfast. So I mixed every spice in there, for flavor. I put peppermint flavoring in by mistake, so they had peppermint flavored scrambled eggs and burnt toast with garlic hot chocolate.....yeah. Should have tasted things first.

I ate a saltine cracker out of an unopened box that had expired a year ago. I didn't read the expiration date. It tasted like smelly cardboard. I came close to barfing in my soup.

I ate jalapenos with my hands, then played with a baby lab rat. He was not a happy rat. He licked himself, then drank a ton of water and ate up all his rat pellets. He recovered by the next day, though.

I was finishing up my manicure, with what I thought was the oily stuff that helps polish dry faster, and generously put clear nail polish all over my nails, cuticles, and finger tips.
