Probably not, unless you pushed through it with something sharp. Even then, it would probably bleed a little the same way any other skin on your body would. Your bellybutton is sorta like knot. With some people, it smooths over better than others. Mine is almost shaped like a diamond in the middle, but it's definitely an innie. Look at how some people pierce their bellybuttons, the ring goes through the upper lip of their navel and through the center. See what I mean? I wouldn't worry about it, whether you're an innie or an outie.

My sons belly button opens and bleeds periodically. The doctor said it probably never totally shut but it's odd to me that it never did that when he was little.

you can get a hernia of your umbilus(bellybutton) which allows the fat and sometimes the intestines to cause your BB to protrude or stick out. this is pretty serious and should be corrected by surgery but the skin is usually able to stay together. (in other words, you won't burst and loose your guts)

It can be stretch. But It's no more likely to open up than any other part of your skin.
