It depends on what kind.
A portable one has a razor blade next to an angled opening that shaves the pencil down until it is pointy
A wall-mounted manual sharpener uses two grinders to shave the pencil down when you turn the crank.
An electric sharpener does the same sort of thing, but you don't turn the handle, pushing the pencil in activates an electric motor that does it for you.

A pencil sharpener works by removing the wood in incremental amounts exposing the graphite and creating a point so that it glides across the writing surface.
