In answering this question, we have to make some assumptions:

a) the traveller is in US or any other part of the developed world;
b) the person is not Indian or has been mostly abroad and used to the western ways of life
c) you are keen to make the person's travel very comfortable and without any hitches. and
d) that the person is likely to visit a number of locations as a tourist

I would recommend a package and not just one item. To put together your package, here is what you might wish to do;

a) Buy him or her one of the travel books such as Lonely Planet Travel books; if the person is not going to many places but will visit only two or three cities, then copy the relevant pages (enlarged to 200%) and share the same;
b) A bottle of chlorine or water tablets to help the person drink safe drinking water; although bottled mineral water is available in most locations in India, with spurious bottled water also being rampant, this may be one way of ensuring that the person is safe;
c) a good mosquito repellent cream - ODOMOS that is available in India is generally good too.
d) A wallet to hold various items - passport, ticket, credit cards (which will allow the person to withdraw money from the hundreds of ATMs in India directly in local currency from their accounts in US or elsewhere), key addresses and phone numbers, currencies of India as well as of the parent country;
e) A small medicine kit, with the common medicines that the person normally takes for simple items; e..g. you dont get Tylenol in India, but get Paracetamol; you get most medicines cheaper, but the local names of the medicines are always different and your friend might constantly struggle; do remember to add something like Imodium, if your friend is an adult and has a tendency to have a poor digestive tract.
f) and above all, a good card for the digital camera - meaning something like a 2 GB smart card, because there would be just too many sights to click and it may not be always feasible to transfer them to his or her computer promptly before taking more photographs.
