Oh lordy, that a bad one. I assume you've tried your best to scrub and clean it? If not - start with that.

Then go to the pet supply store, and get some of those "Nature's Miracle" products (there are several - one has that name). It's a cleaner with living enzymes in it that "eat" proteins - and it consumes the proteins that rot and stink - usually people use it for treating pet messes in the house. But it will work for milk stink, too.

I have the perfect solution after you shampoo the carpet. It is a product called FRIDGE IT odor absorbers. It is a little purple cube with an activated carbon filter inside that is really great. It works so much better than baking soda and all those sprays and fragrances. I'm not particulary fond of fragrances that just cover up odors and do nothing to get rid of them. Also, most have warning statements on them as eye irritants. Just put a couple of these odor absorbers in the car and also trunk.. No messy pieces or carbon powder to worry about. The carbon filter in this product literally eliminates the odors and works for a long time. I use this product everywhere including my fridge and freezer and also under sinks, near trash, closets, drawers and have friends that use it on cat litter box and also diaper pails - it's that good. I really recommend it and it's not expensive. You can buy it in kitchen gadget sections in stores like walmart and linens and things. I also think camping world sells it. Definitely worth trying. It's the best product for any odors around the house and car!

X-O Cleaner!!! It gets rid of ALL odors!! I dropped a venti hot chocolate down the side of my chair and center console. I took the concentrate and diluted it to the correct strength listed on the package and poured in right onto the carpet. Within seconds the odor was gone... and it hasn't returned since (it's been 6 months)

Wash the carpet.

Use Febreeze Auto; works like a freakin charm! I think it even smells better than regular Febreeze. Anywho, you can find the Febreeze Auto in the automotive section where the car cleaning soaps, sponges, etc are.

I would dampen the carpet with a mixture of hot water and laundry soap, then scrub it, then step on it with a towel to soak up the water deep in the rug, the sprinkle baking soda on it let it dry then vacuum it.
