Technically, Cynthia and Jason are both wrong and right. There were numerous cards of which only one copy was made. The ones that were left out were Splendid Genesis and Fraternal Exaltation, these cards were made to celebrate the births of Dr. Garfield's children. Proposal was never printed, but photcopied and glued to common cards in a very small quantity (rumored to be four), only given to their closest family friends.

I'd say 1996 World Champion is the rarest card in Magic. Only one was ever made. Here's some info on it-http://www.wizards.com/default.asp?x=mtgcom/arcana/91
