if you can take the shower head off soak it in a container with a 50-50 mix of hot water and vinegar.
if you cant remove it pout 3 inches of vinegar in a sandwich baggie. put the baggie over the shower head and secure it with duct tape and let it soak overnight.

Vinegar in a big plastic bag, put it up around the shower head so that its submerged in the vinegar, rubber band the bag to the shower head and let it sit for a few hours.

Put some Lime-A-Way in a small baggie and attach it to your shower head using a rubber band. Leave it on there for about 15-30 minutes remove baggie and run shower. That should do it.

Just dump it in some vinegar. Rinse well. Twice.
