I went to guest service in the Magic Kingdomand got a Its my Birthday button. The cast member on duty encouraged me to wear it the entire weekend since the whole trip was for my birthday. We had such a fun time with all the cast members. We were asked to grand marshall a parade in Animal Kingdom, while waiting for Spectromagic one night a cast member led the crowd in singing Happy Birthday, characters acknowledged it, ride attendants acknowledged it. It was fun. I can only imagine it would be an even bigger effort for a child.

When in Disney why don't you book a character breakfast in Magic Kindgom where the characters talk to the children whilst they eat. The kids love it and if you explain to someone on arrival that it is a birthday surprise you might get extra special treatment.
I'm sure you can arrange in in advance on the website. If you are staying at a Disney property you can arrange it through your hotel and charge it back to your room.

If you're staying on property, tell the hotel concierge and they will help you make it more special. If you tell them before hand they will have things waiting in the room for you. You can have a Mickey Mouse cake delivered, they'll give you a Goofy birthday wake up call, and you get a button that tells everyone it is your birthday. Cast members always notice these.

Make sure that as soon as you get there you go to guest relations and let them know that it is your child's birthday. They will give the child a button to wear and throughout the day cast members will pay special attention to your child.
I'm sure there are other things, but the button thing is no extra cost.
