For about 3 years now I've been experiencing very heavy periods, often with heavy clotting. The huge clots are what worries me most. They're anywhere from the size of a half-dollar (small) upwards to fill a 5 oz. dixie cup.

In Feb '06 my GYN sent me to the ER due to the clots with heavy bleeding. The ER did absolutely nothing... it was a waste of my time to go there. I followed up with my GYN 2 weeks later and again in July '06. She has no explanation for the cause.

In July '06 I was diagnosed with Type 2-Diabetes (A1C at 10), and high-blood pressure (220/190). My Internist has worked with me to get these life-threatening conditions under control: A1C now 5.8 and BP at 120/74. I discussed my heavy periods with him. He ordered tests to check my hormones in addition to the regular blood tests I have every 3 months. All of my hormones are within normal range. I saw him in Jan '07 after the tests... he also has no explanation.

Can anyone offer some idea and treatment?

I am 47 years old, married 27 years, two adult children. I struggle with depression and am often tired. I exercise daily, walking 4+ miles and light weights.

I have annual PAPs every June/July. There is mild pain in my low back for 2 days during the clotting phase of my periods, not enough to take pain meds. In '94 I had a miscarriage and shortly afterward needed cryotherapy for dysplasia. I had a D&C in June '05 following "abnormal squamus cells" PAP, an internal sonogram and IUD removal. In July '05 got a new IUD after a normal PAP.

The IUD I have is Paragard-T... the one with no hormones.

When you're in perimenopause, you often ovulate and you still are producing hormones at higher than post-menopausal levels. In fact, most often, you'll be producing hormones in the normal range -- one of the reasons this can be a difficult condition to diagnose.

In response to your added details about the IUD:
Yes, an IUD can cause heavy bleeding with clots, but that is just one possibility. Until you decide what is going on, you should be taking a multivitamin with Iron. Good Luck, sometimes menopause is better than pre-menopause.

Number one, that IUD could be causing your clotting and should get it removed or at least discuss it with your doctor, but like you said, your getting no where there, right. I have this same clotting problem as you and normal test across the board, but something is being done to help me. I find out next Monday if they are going to do a DNC with a touch of burning, to lessen the bleeding or flat out a hysterectomy.Did any of your kids have nose bleeds as a child? Did they have to burn a vein in order to stop that weak vein from breaking, well sort of the same thing applies here with this DNC with cauterization. You have options no matter what your insurance is. Now that I have Medicare, doctors are willing to treat my symptoms, when I was on medicaid, they(doctors) said duh, and years went by while I have endured this pain. I am now 36 years old.I wish you well, you sound like your at your Witt's end and I DO know what that is like, believe me. I meant what I said about that IUD, get it out and do something, even if that means seeing another doctor for a second, third, even fourth opinion. Enough is enough, your the boss of your body, feel free to e-mail me with other concerns, due to I have been going through this for 7 years now, yep yuck, it sucks!! I might get rid of it all, just not worth it anymore.

I will assume you have annual exams with pap smears that show no abnormalities. Are there any other symptoms- pain, fever, abdominal/pelvic distention, tendernss etc.? or are the clots the only symptom? Need more info. But I would suggest a second opinion by another gyn if this has been going for 3 years and no results.

I know this upsets you, to me it sounds like you need a D & C.
There are a few reasons this can happen. One, is a spontanious mis-carriage. Did they due a Sonagram, or a MRI Scan? If not, you need to go to a different OBGYN. Prendnizone can be prescribed for this too. Keep, going till you find a Dr. that is Board Certifyed in your area.
Nothing can be diagnosed properly here. But, you do need help.
