I just got married in Vegas, and live in Chicago, how do I go about in changing my name?

1) Get on the Social Security website, print out a change of name form, fill it out and take it to your local Social Security office with your current SS card and marriage certificate. You'll be mailed a new SS card.

2) At work you need to submit a new W-4 with your new name (and address if you're moving).

3) Take your marriage certificate to the DMV and get a new drivers' license (make sure you change the address if applicable).

4) You need to call your creditors, alumni assoc., etc. and give them your new name (and address).

That's why we don't make the men take OUR last names!

You just contact the proper offices, go to the drivers liscense bureau with a copy of the marriage certificate, the same with your bank and any other place that you maiden name is used. You can call the Social Security office or go to your local one, and request a form to change your name, they also will want a copy of your marriage license.

Once you get your marriage certificate you can go to the social security admin. to get a new card (will me mailed within 2 weeks) and go to the dmv to get a new license, usually if you go to the dmv within 30 days its free to have reissued.
