I am thinking of buying one as a gift. So I would like to know if there is something I will need to buy extra.

Depending on how you specify it, you could well not need anything else - every Dell laptop I've seen (a fair few!) comes with a battery, CD of Windows, power supply and power lead.... you just need to make sure any software you would like is added to the specifications, and a spare battery/bag or anything else you'd like to add...
But it will work straight out of the box (o:

If your buying a Dell, you really should check on their website. I'm fixin' to buy one next week and go there everyday to customize and check prices for what I want. Its real easy to walk through and shows you upgrade options step-by-step. Everytime you upgrade or downgrade, it automatically adjusts your TOTAL and the price of the up/downgrade. They also show what they recommend (remember they are in the sales business). Since you are buying a gift, I assume you might buy just a basic. Beware that their cheaper notebooks come with a 4-cell battery which last roughly an hour. I strongly recommed upgrading to the 9-cell (there is also a 6-cell available). They have tons of options, so be sure and visit the website!

I would think a protective case of some kind should be an immediate item.
There are all sorts of small items you can add, bluetooth is one of my favs (for my phone and printer). Try to get this built in.
If the person is on battery a lot, then a second battery.
Make sure it can burn DVD's for backup.
And a USB thumb drive, like a 2gb is a very handy tool.
