The UN General Assembly is made up of representatives of all the member states. I have pasted a link from their website below.

There are also a number of non-voting observers in attendence at the general assembly. I have pasted a link to a PDF file that lists those observers.

Yes. The U.S. is also a permanent member of the U.N. Security Council. It was one of the original 51 members of the U.N. in 1945.
