Not only sparks from the ignition system but the hot exaust could also start a fire. Most cars now have the gas door on the opposite side from the exaust. Also on the new cars the gas tank must remain sealed while it is running or the computer will read a fault and you will get a "check engine soon" light on the dasboard. In my state (MA) part of the state inspection is to check that the gas cap is functioning proper.

OK besides all of the blowing up stuff here(possible but not real likely) today's cars are made to burn not just the fuel but the fumes as well. This is how they manage to get better fuel economy. Running the car with the cap off of the inlet lets out fumes that the computer registers as a fumes/fuel leak and can trip the monitors in the cars computer to register a fault and hence kick on the MIL (monitor indicator light) on the dash.

2 reasons so you can't drive off with the hose still in the filler. It dose happen. The other is while your putting gas in you car running. It wouldn't take much for someone to steal it

Because the spark from the ignition coils and spark plugs and wires could ignite any gas fumes coming from the area of the tank while filling up.

you can if you can live with the possible results of seeing your car burning because of the fire that was started by a spark from the engine. No it will not happen every time , but it can happen. This is a safety issue
