Besides cities be bombed in to the stone age the answer is Yes,

There were at least two towns in Europe and enumerable towns in Russia that were wiped off the map as reprisals by the German's for partisan raids There was a town in the old Czechoslovakia where the Germans killed every man in the village of Lidice because partisans machine gunned and killed the SS commander in Prague. The was a village in France that was shot up by the SS Das Reich Division because they couldn't dodge Allied Warplanes in June 1944.

The Japanese would destroy whole villages in China just for the hell of it. The Second World War was not nice it was be most brutal and heinous type of war that this world had seen to date.

No, despite the worst of the devastation in Hiroshima, Nagasaki and Dresden (for examples) there were survivors and there were buildings standing.

I know of one particular case of slaughter where a town was not re-occupied and restored, but that was as a reminder and accusation:
Oradour sur Glane remains much as it was left on 10th June 1944

Lidice, a village in former Czechoslovakia (now in the Czech Republic). In June, 1942 at the hands of the Nazis all of its citizens were murdered or sent to concentration camps. The town was thoroughly destroyed. A memorial stands in its place.

No. Also, Nagasaki and Hiroshima are not towns. They are cities.
