When he first turns into a beast, shoot him in the head (the diamond) with your bow. Be sure to move away before he crashes into you. Slash at his gut then. Eventually, he'll disappear after being hit in the head for a while. That is your cue to go wolf. Face the portal he will come out of (it turns blue), and as he comes towards you, press A to grab and throw him to the side. Then chomp (or slash if fast enough) at his belly again.

fight him in your wolf form is easier, use minda's hand to flip him over and then bite his underside(3 times)
human form hit him with an arrow in the head with he comes out of the twilight. he falls down and slash him on his underside(3 times)
