Not in the world - mostly in the US. I've lived several other places around the world/US and the fattest people I've seen by far have been in the Midwestern US. I really want a bumpersticker on my motorbike that says "Eat less and exercise more, dammit!!!"
Poor diet/eating habits, lack of portion control, drinking sodas or other high-calorie drinks throughout the day instead of water, little or no exercise and delusional thinking all contribute. The diet Coke does NOT cancel out the calories from the giant wedge of chocolate cake you are shoveling down your maw! I can't stress that to one friend enough.
Honestly, I think those obese people who do not have medical conditions (which are not as common as the obese would have you believe) are, for the most part, addicts. They are addicted to food just as surely as a smoker is addicted to nicotine, an alcoholic is addicted to alcohol and a crack head is addicted to crack cocaine. First they need a medical work-up to rule out any medical problems, then they need help. If they cannot help themselves, they need to look for an Overeaters Anonymous and/or a doctor's guidance. Their lives are at stake! It is a slow suicide, but suicide nonetheless.
I know it isn't easy. I've watched friends suffer through it in the past and am watching a few struggle with their weight now. I just really hope each of these people who is struggling with their weight will start keeping an HONEST food diary, eat a bit less, eat healthier and start exercising more. You'll feel so much better about yourself!

primary reason- Genetics. Whether or not someone is born to obese parents they can still become overweight if they have certain genes that enable it. Then again there are also those who have two obese parents and have no problems at all with their weight. Relatively speaking, I have found that the later a child is born to an obese parent the higher chance they have of being overweight. For example an obese parent may have 2 children with no weight problems, then have a third child who is genetically overweight. But really it just depends on the person and his or her genetic makeup.
Secondary reasons: Lack of education/awareness. As humans age they tend to become exposed to an environment where there eating is less regulated and more difficult to control. As children most humans are able to eat only what their parents give them or whatever is available. But as they become adults they enter an environment where they suddenly have to be in control of what they eat. Unfortunately for many people they have not been informed about proper eating habits, or they simply choose not to make informed decisions when eating. In addition, although they are better for you, healthy foods tend to be more expensive, and many people don't want to eat them anyway. These people more than likely figure there is no need to spend more money to purchase foods that they don't even want to eat, which results in them buying the less expensive junk food and the like and only contributes to their obesity if they are prone to gain weight.
Hope this helps.
