what kind of health issues should worry about?

There are NO health issue here other than bring a lot of suntan lotion and some bug spray.
I love it here.

Been twice. Both times with young kids , no health issues, no vaccinations.Beautiful country , friendly people, gorgeous resorts. Would move there tomorrow if could speak Spanish /.

Just don't drink tap water and eat junk on the streets and you should be fine. Always bring Maalox and Immodium my 2 favorites when travelling to DR just in case. My favorite Presidente Light!!!!

Make sure you are healthy before you go, so you can drink a lot of Presidente.
