Does it mean you do something wrong once, twice, 3x, or something you've been doing for months?

It has two , count 'em two meanings to me. If a human being makes a mistake and can't blame it on a machine or mother nature, it's human error, and it only has to happen once--as in pilot error--(to err is human).
Or, if one believes there is a god, it could be said that when he created humans, he made a grievous error, and simply won't admit it. Humans are the only animals on earth who blush--or have reason to. Gluttony, wanton killing, pollution, (to err is human).

Human error just means that we are not perfect and we will always make mistakes throughout our lives. So, we are all better off to accept the fact that no matter how hard we try, sometimes we will fall short of what we expect of ourselves and others. But it is not reason to give up and stop trying nor is it reason to condemn others for their short comings. Also,though, it is not to be made as excuses for ones lack of trying either.

It simply means you can't blame anything other than a human for something which has gone wrong.
