To shorten the process of how it goes (this is VERY general and will vary slightly):

You go in, fill out paper work and talk with the piercer about what you want done. You'll go into a private room and they'll have you rinse out your mouth and maybe brush your teeth (you should have done that before anyway). From there, they'll get out their tools, explain to you what's going to happen and what kind of jewelry you are going to get. Once that's done, they put the clamps on your tongue and check the area for veins. As soon as they determine its safe to pierce, they get the needle ready, tell you to take a deep breath in and on the exhale, they pierce. From there, jewelry is inserted and you are to rinse your mouth again. Before you leave, you will be told aftercare instructions and should be given a sheet with this information on it, it should also have the name of the person who pierced you as well as phone number and address for the shop with operating hours

Toothpaste probably won't burn, but I always suggest that people use kids toothpaste for a few days since its more gentle. And don't use regular mouthwash, even diluted, it could cause issues and prolong swelling and healing. Biotene is best because its alcohol free and is very gentle, though it does taste a little odd.

The needle is a straight hollow, beveled needle, usually 14g. The jewelry put in will be a surgical grade stainless steel or titanium straight barbell with a longer length to adjust for swelling. Once swelling goes down and usually about 2-3 weeks after, go back and they will give you a better fitting barbell.

Your tongue will swell and you may have difficulty eating and talking for a while. Take your time eating so you don't bite your tongue or the barbell and be sure you clean the piercing as instructed.

NEVER trust anyone that uses a gun to pierce anything.

They use either a 14 gage needle or a 12 gage needle and the jewelry is put in the needle so that when the needle comes out, the jewelry is in place and all they have to do is put the ball in the barbell. Many places use a slightly different technique but it's basically the same thing. Some places use a numbing paste but it's not necessary as it's one of the less painful piercings you can get. Toothpaste may burn a little but it's nothing. You need to swish with salt water and or cool mint Listerine after you do anything with your mouth such as eating, smoking or even kissing.

They put a clamp on your tongue so you can't move it, then they use a 14G hollow needle, put it through your tongue then put the jewelry in. Guns aren't used for body piercing. They will give you cleaning instructions(each place has it's own idea of what the best way to clean the piercing is), but usually they give you an antiseptic rinse and you are supposed to rinse with that several times a day. Not sure about the burning, as I've never had mine done.

I've had my tounge ring for almost 3 years now so I think I can help.
The process for getting it done is simple. Just go out to a tattoo/piercing parlor (one that is well-known and people give a good recommendation for) and let them know you want a tounge piercing. It usually costs anywhere between $30-$50. They'll have you sign a few papers warning you of certain things that could happen and telling you the parlor is not responsible if something bad does happen, and then sit you down to get ready for the piercing. The first thing they do is take a paper towel and dry off your tounge and then they'll take a little marker to mark where the piercing will go. Then, they'll take some tongs (sort of like the kind used for barbeque but a LOT smaller, obviously) and put them tightly on the center of your tongue and pull your tounge out as far as possible (it doesn't hurt, you'll just feel pressure from the tongs). They then take a 14 gauge needle and quickly put it through the holes in tongs making a hole in your tounge where the bar will go (there is no gun used in tounge piercing). It didn't hurt to me but I have heard people say it hurt them. The bar they put in is a lot longer than regular tounge ring bars because your tounge may swell. Keep that bar in for 2 weeks (or however long they tell you) and eat a lot of ice. You'll have to Listerine after every meal, after every drink, and if you smoke after every cigarette to keep infections away (you can do like I did and keep a small travel-size bottle of Listerine on you at all times like in your purse). Toothpaste won't burn but brushing your tounge will feel pretty funny the first few times. When the 2 weeks is up you can go out and get a regular sized bar and change them out. The parlor you get it done at may even have some bars for sale.
As for the risks: make sure you go somewhere well renowned for these sort of things. If they are off a quarter of an inch when they pierce it, it could damage your tounge forever making it so you can no longer taste or even having to get it removed.
If you want to keep the piercing for a while, wait a few months before you take the bar out. I waited a year before I took mine out for any amount of time.
Good luck!

Its a hollow needle, and they slip it through your tounge and then they slip th jewelery ... your tounge will swell up to the point you can't talk for about 2-3 days... really easy to care for, no pain in the peircing process... get it done!
