If it is strained it will heal with the proper exercise. stretching and strengthening the whole shoulder usually with rubber bands. It is a slow process. If it is actually torn, then surgery is the only option. The difficult part is to properly diagnose it. It can be partly diagnosed with an MRI. X rays don't work on soft tissue except with die. You will have to find a good surgeon that specializes in shoulders. A shoulder is very complicated and in several layers.
I had a torn rotor cuff repair six months ago. (tendon torn from the bone) It is the most painful surgery there is and you have to wear a sling for three months and can't drive a car. Then you have to build the muscles back up. plan on six months. during that time the shoulder has to be manipulated through full range of motion several times a day or it will freeze. I am ok now.

The most common symptoms of rotator cuff injuries are pain and weakness in the damaged shoulder. Sometimes a person feels pain only when using the shoulder; for example, to reach for an object or to lift something overhead.
If the rotator cuff is torn completely, though, the pain can be constant. A person with a completely torn rotator cuff may not be able to lift the arm from the side of his or her body.
People with rotator cuff injuries often experience pain at night because sleeping in certain positions puts pressure on the shoulder. In some people, the injured shoulder makes popping or clicking sounds when moved. For others, the shoulder seems to stick momentarily.
f these early warning signs are heeded, specific steps can be taken to resolve the problem without surgery. That's why it's important to see a doctor if you notice severe pain in the shoulder or are unable to use the joint properly.
NONSURGICAL treatments of rotator cuff injuries include:
Rest. The first step in treating any rotator cuff problem is to rest the injured joint until the pain and swelling subside.
Ice. In the first 24 hours after an injury, ice can help reduce pain and swelling.
Heat. After 24 hours have passed, a heating pad or hot compress can help an injured rotator cuff to heal.
NSAIDS (non-steroidal anti-inflammatory drugs). NSAIDS such as aspirin and ibuprofen can reduce both pain and swelling. Be careful to take NSAIDS as instructed, since they sometimes can produce stomach upset and gastrointestinal problems.
Physical therapy/exercise. Many rotator cuff problems can be successfully treated with gentle exercises designed to stretch and strengthen the shoulder muscles. Usually, it's best to begin physical therapy as soon after an injury as is possible. Avoid lifting heavy weights overhead, and avoid activities such as throwing a baseball that put a significant amount of stress on the tendons.
Steroids. Steroids such as cortisone may be used to reduce inflammation. Steroids can be taken orally, but for rotator cuff problems they are usually injected directly into the area around the tendon. Steroid injection should be used carefully and multiple injections should be avoided.

A torn rotator cuff is extremely painful and just about totally immobile...believe me, you would know it was torn (your doctor would have had a scan done and diagnosed you). You can also pull your rotator cuff, which has happened to me and it causes pain with movement and weakness in the shoulder, so you cannot lift anything as it feels like your shoulder will pop out of its socket. If your rotator is torn you sometimes will need surgery, if it is pulled you will need alot of rest and a few months of physical therapy but it will heal. I pulled mine 3 years ago in a car accident and it is almost back to normal...I just sometimes get that feeling of weakness when lifting something heavy.

Dangle your arm. Do not let your shoulder stay in the same position. Your shoulder will literally "freeze" and will be immovable. This is called "frozen shoulder." To prevent this, dangle your arm loosely. This will put weight on the shoulder. This will keep the shoulder mobile. It will also reduce pain.
