The pin only holds the spoon on the grenade. Yes the real name of the lever is the spoon. The spoon is actually the trigger, so as long as you don't release the spoon the grenade will not go off. Once the spoon is released the grenades fuse starts and this is where bad things can, and occasionally do, happen.
That said grenades are mechanical devices and as such they can go off, quite by accident, particularly when the explosives get older and less stable. Which is why the army gets rid of old ammunition, the explosives and powders become highly unstable over a long period of time.

Not when you pull the pin, but possibly when you release the lever. A deffective fuse might cause it to explode instantly.

No.the lever is key..when you pull the pin,you are only making it possible for the lever to go down.
