There are many signs....
"Goopy" eyes, sneezing, and wheezing are signs of a respiratory problem.
Sometimes, this can be resolved by putting their food into a bulk feeder with a dust screen on the bottom. Rabbits have sensitive respiratory tracts and the dust can cause breathing problems.

Loss of appetite, excessive appetite, fur missing in patches, lack of activity, cloudy eyes, loose stools and very smelly urine are signs of an internal problem.
Check their food and water supply, ensure they are not caged together so any vector disease (kinda like colds or flu and parasites) cannot spread. Rabbits will fight, sometimes to the death, so try to keep only one animal per cage.

Have your animal checked by a vet for a definate diagnosis.

You must come to the realization that you may have to put a very sick animal down as treatment options can get expensive for them, but I understand that pets have high sentimental value. It's up to you, as their owner, to do the right thing.

A good sign i have found is that when its lay on its back with its legs in the air not breathing, that is usually a good indicator that something is not right.

It also depends on what you mean by sick, if the rabbit is trying to get off with its sister, that also is pretty sick.

cloudy eyes
runny nose
laboured breathing, gurgling sound when breathing
loss of appitite and weight
reduced mobility
less active
runny stool(poo)
blood in stool, urine or cage
strong odor in cage(stronger than normal in a clean cage)
drinkin/urinating more often than usual
difficulty urinating/passing stool
little white worm things in stool (maggots. it can happen. pretty nasty)

if you suspect any illness see a vaet asap. If they try to give you oral medication, argue with them untill they give you injections. it is very hard to medicate a rabbit that is VERY sick. the one living here could not breath and could hardly move. anything that was put in her mouth would just dribble out. she almost died but had a compleate turn around within 2 days of injecting the antibiotics

If you suspect that your rabbit is sick, please be sure to find a good veterinarian. Here are some of the symptoms:

Loud tooth grinding
Abnormal Body Heat - particularly note the ears
Runny eyes or nose
Labored breathing
Chronic sneezing
Wet chin or drooling
Loss of balance or head tilt
Loss of apetite or lethargy
Irregularly shaped droppings

usually the first sign is when the rabbit goes off feed and water.

Runny nose and always pawing at its nose, watery eyes with discharge, difficulty breathing.
