Miriam Webster defines literate as:

Main Entry: 2literate
Function: noun
1 : an educated person
2 : a person who can read and write

So if you are a health literate person (a health educated person) you can apply your knowledge to your own lifesyle, using your abundant information on the subject to "achieve and maintain good health."

Pretty simple, huh?
