Yes, sometimes they get pinholes. When put on they have to be all the way to the base, no air pockets and a little room at the top for the sperm.. They can slip off, break or even make little tiny holes that you don't know about...

Yeah if the condom breaks, sperm slips out, etc etc.. I have def. heard of people getting pregnant although they used a condom. Do some research on it and you will see yourself =)

YES. They can break, or sperm may "escape" before you both manage to get the condom on. Some men are not good at removing the condom properly, and some sperm may drip out!

Read the box. Yes you can it is not 100% fool proof. No sex is the only way to be sure you will not be with child.
