Life as we know it would be over for sure
To begin with, it would probably cause a massive winter over the planet. Areas hit by the blast would be depopulised dramatically. Most of the world's population is in the cities. Radiation would melt the skin from your bones. There is footage of the chernobyl disaster taken from a cameraman in the zone - he died one hour later from radiation poisioning. This was a fully grown, healthy, military-trained man.
The radiation would also cause terrible pain and deformalities. Your hair and teeth would fall out. You would bleed from every orifice, like ebola, but much worse. Your blood would become toxic. If you didn't die from the fallout, you would be in for a lingering and certain death anyway.
Crops would die, there would be huge food shortages. Law and order would break down from panic and there would be uncontrollable riots. Military law would be imposed in such areas and there would be a shoot-to-kill policy. Armies close to hit areas would mutiny and flee. There would be no more doctors, no more hospitals, no more shops, no more cars. Transport would be impossible altogether - a large nuclear war would disrupt the earth's magnetic field, hampering electronic equipment, maybe even send planes falling from the sky.
The rural areas would mostly perish, like in the way the Black Death hit europe, and develop their own law and order. Disease would flourish. Even areas completely unaffected depend so much on international trade - you would probably hear a lot of extreme right-wing sentiments and many would be killed in the thick of the chaos just because of undisputed grudges or whims. People would probably turn to religion in sheer desperation and survival would depend on whether you were part of their 'flock'. For instance take a look at what the Nagasaki and Hiroshima bombs did to Japan's collective nationality.
Not forgetting, of course, the damage to the animal kingdom. Cockroaches, rats and other household rodents are capable of surviving high radiation. You would have literally clouds of insects descending upon you. The only place life could continue as before would be at the bottom of the mariners trench.
No more schools. No more TV. No more internet. No more food. No more water. No more tomorrow.
