It depends. I can help but you need to answer a few questions so I can try to figure out what went wrong.

How long has the tank been set up?

What kind of fish?

What size tank?

How did you cycle the tank?

What is your water change schedule?

What do you feed it?

Unless you can answer these questions, there is no way anybody can give you a realistic idea of what happened. All anybody can do is guess.

Sorry

There could be a number of problems here.

Fish was unhealthy at time of purchase and the stress of taking it home could have just increased that.

Your friends water could be bad. Tell your friend to take a sample of her tank water, about a cup, to a local aquarium store. They normally will test the water for free and tell her if she has any probelms. Do this before buying any new fish.

Also most fish other than gold fish, betta's, white clouds, need their water temp to be above 76 degree's if she doesent have a heater in the tank this could have been the problem.

Sudden change in water temp. Always float the new fish in it's bag from the store for about 15 minutes in the tank before letting it go, so it can acculmate to the change in temp.

And sometimes some fish just get stressed during the trip home, fish can die very easily from stress.

I hope your friend has better luck with her next fish.

NEVER, EVER get fish without doing some research(if you haven't, it sounds like your friend didn't know much about keeping fish). Keeping fish is not as easy as many people think.
I doubt it was underfeeding, most beginners go the other way and overfeed. Tell your friend to make sure that the water is free from chemicals that are usually found in tap water (water from a sink or faucet). Also keep the water clean by changing it often.

She probably died because maybe she wasn't getting enough food. Or the tank wasn't that clean. She might have had ick to. It is when they have little white spots on their fins. That is all I can really think of. Sorry!
