The big problem is that the onions that Taco Bells gets are grow by their own company and so when one of the restaurants had a problem with the onions all of the franchises had to stop serving them for a while in case of a lawsuit.
Hope that cleared some things up.

Be glad your salad didn't have green onions! Check this out:
Taco Bell has decided to recall all the green onions from all of its 5,800 locations in the US after an outbreak of at least 43 cases of E. coli infections in New York, New Jersey, Delaware and Pennsylvania last week. Testing revealed that they were a possible source of the bacteria, although other non-meat sources have not been ruled out yet. Samples of the cilantro, cheddar cheese, blended cheese, yellow onions, tomatoes and lettuce are being sent to FDA labs for testing and comparison to the green onion samples. The states were the infections occurred will be testing their own samples, and Taco Bell is likely to have scientists at work on their behalf, as well. The sources for the potentially contaminated foods are still being identified, but consumers (unless you just ate at a Taco Bell despite the potential health risk) should not worry too much about it because suppliers for large buyers such as Taco Bell rarely sell directly to supermarkets.

There was a recall on green onions when someone got food poisining from what they thought was the green onions as it turned out it wasn't from the green onions at all more testing showed it was actually from the lettuce. This was in the paper third page after the spinach scare happened. I work in a restaraunt as a server and we also pulled our green onions until we knew it was safe.

There were some problems with Taco Bell veggies of some sort a few months back. It was a big joke on the David Letterman show where they dared people to each food from there. Maybe there was some sort of recall involving those ingredients.

Food-safety fears revived as Taco Bell pulls onions
Video: E. coli outbreak
By John Schmeltzer, Tribune staff reporter. Tribune wire services contributed to this report
Published December 7, 2006
As Taco Bell Corp. pulled green onions from all of its 5,800 U.S. restaurants Wednesday in the wake of an E. coli outbreak linked to some of its restaurants, questions emerged about whether the current testing procedures on produce entering the nation's food system are adequate.
The fast-food chain said preliminary testing by an independent lab found three samples of green onions that tested positive for E. coli O157:H7, a potent strain of the bacteria.
Taco Bell executives said the tests were not conclusive. But the chain was taking no chances Wednesday when it decided to remove the onions after at least 46 confirmed cases of E. coli sickness were linked to Taco Bells in New Jersey, New York and Pennsylvania over the past few weeks.
"In an abundance of caution, we've decided to pull all green onions from our restaurants until we know conclusively whether they are the cause of the E. coli outbreak," said Greg Creed, Taco Bell's president.
Taco Bell is a unit of Yum Brands Inc., one of the world's largest restaurant operators. It also operates KFC, Pizza Hut, Long John Silver's and A&W restaurants.
Eighteen Taco Bell restaurants, including nine in suburban Philadelphia, three in New Jersey and a half-dozen on New York's Long Island, have been closed for decontamination. Some have reopened since the outbreak was first reported nearly three weeks ago.
Taco Bell's E. coli problem comes four months after E. coli contamination forced the nationwide recall of packaged spinach from grocery store shelves and restaurants. Three people died and more than 200 fell ill in the outbreak that was traced to packaged, fresh spinach grown in California.

When Taco Bell was shut down after several food poisonings, they narrowed it down to the green onions. It was only Taco Bell that had this problem, they found no othe contaminated green onions otherwise...Just Taco Bell

There was a recall on them a while ago, like 2 or 3 months back I think, and the only stores affected were in New Jersey

When I was a Carlos O'Kellys they had the same answer for my missing green onions, as a result I found I liked the item without it better, but that's just me.
