Everyone I know does it under and it drives me crazy. They say it's the "right" or "proper" way. I say it's the dirty way. Think about it this way:
If you do it under, then the paper is closer to the wall. A dirty hand, not yet washed, will have a much greater chance of touching the wall there and thus speading germs.
Done over, you have a wider gap between the wall and paper, giving the hand a larger gap to grasp the paper, without rubbing it along the wall.
This is assuming that you're using the standard in-wall toilet paper dispenser.
I also just think it's easier to roll forward than to reach behind for a new sheet.

Definately over and if someone replaces the toilet roll and puts it under I always fix it up.

I never remember to look when I put the toilet paper on the holder. I just put it on any way. My paper towels are sideways on the holder.
