Well, since there were no movies in the Med. days, they didn't need a soundtrack everytime they turned around. Nowadays, here in the States, people have other things to do besides gather around whatever people who can play instruments and sing. That used to be all the entertainment there was besides plays.

There have been so many changes during the past five hundred years that is would be easier to ask "What functions of music have remained the same from the medieval period to the present?"

None of the orchestral instruments existed five hundred years ago (although there were some forerunners of modern instruments).

None of the current music notation existed five hundred years ago (although there were some forerunners of modern notation).

About the only categories of music that still exist from five hundred years ago are the Roman Catholic mass and folk songs (although most of our American folk songs did not exist until about one hundred years ago).
