Many reasons...He likes the color red...He didn't have any other clean ones...He is trying to match the shirt he is wearing...He has a weird mental disorder and thinks that if he doesn't wear red underwear he will be abducted by aliens...He doesn't care what he wears he just grabbed something and put it on...he has Valentine's Day spirit...he thinks the color red looks good on him
If you are thinking he is gay b/c he is wearing red underwear...find a better reason

It means that he likes red... if it's red women's underwear it means they're his girlfriend's and the two of them are having a little fun, or it means he has transvestite fantasies.
It could also mean that they were once white and have been put in the wash with a new red jumper.
