very very fake!!
of course it is preplanned....
it helps the ratings by having things lined up that will appeal to its watchers. otherwise, it will be like boring. they make sure that when the cameras are rolling...its filled with some drama, excitement, funny, etc. in order to keep people watching.

Sports seem to me to be the only form of TV that is actually reality, it is usually live, spontaneous and unscripted or edited. These other shows that claim to be reality are extremely edited and the participants ham it up big time for the cameras.

Producers & writers manipulate most of it. They know what viewers want & don't want, they know how to create controversy. There's a show on VH1 called Reality TV Secrets Revealed that exposes alot of the reality tv shows and shows just how unreal they actually are. They repeat it now & again, but here's a link to it on the VH1 site.

There is no such thing as reality t.v. - it is all fake.
Producers, and t.v. networks have the last say on what really happens... it's all a bunch of nonsense if you ask me - but what the heck... people love it!
