Personally, I really like the Ritz-Carlton. The San Francisco location's opulence is not "fake", for one. The structure is actually a converted 100 year old bank headquarters. The outdoor Sunday Brunch Buffet (accompanied by live jazz) is hard to beat. Rooms are what you'd expect for a Mobil/AAA five diamond establishment. And the service is quite attentive.
However, other worthy contenders for the luxury title include the Mandarin Oriental, Four Seasons, St. Regis, Clift, Campton Place, W, St. Francis, and Fairmont.
There are also a number of smaller boutique hotels and B&Bs that are technically not five-star, but are widely loved. These include the Triton, Monaco, and the old Archbishop's Mansion.

I'm particularly fond of the Fairmont and the Mark Hopkins (both on top of Nob Hill).
