Lack of "education".... and
All the crap and guilt that we have fed since birth by several "systems" of exploitation of the masses (state, religion, interests)
In Crete Island In Greece there is a grave.
Nikos Kazantakis' Grave. On his grave stone is written:
I don't Fear Anything.
I don't Hope Anything.
I am FREE.
He really was a TRUTHFULLY FREE MAN.

I'll take a stab at answering this.
A lot of times the truth is hard for people to take. If you or I happen to be in the unlucky position to have to tell someone the truth about something that we know will make them unhappy, that would be where being fearless would come in.
Also, having to face the unpleasant truth about something is not easy. These fears can take many forms in many different areas depending on the subject.

One of the root causes for our fear is self preservation. A great fear in man is for the loss of his life. This feeling has helped to preserve him throughout ages and to preserve civilization.
However historians like Will Durant mention that early Christians were willing to die, eaten by wild animals in Roman circuses, rather than deny that they were Christians. They were truly fearless in front of undeniable danger to their lives and preferred to be truthful rather than to be safe.
This heroic act could be considered nowadays by many as fanaticism but it was done consciously out of conviction and love for mankind and probably also out of respect for their own person and what they believed.

We do experience anxieties in life. This behavior is normal and useful, because some degree of urgency is required to perk the concentration into proper perspective. Like in a job interview, or getting something done, the seriousness is imaginable when focus the only concern. Thus, an anxiety in the natural form is precondition in the development of our senses and personality, as one unifying force toward objectivity. However, on a distraught level, anxiety in their diverse form of activation, degenerate the health of both physical and mental which overtime will rupture into disorder of fear. Other defects, such as phobias, depression and panic attacks are common to the recipients who have experienced the fear syndrome.
Occasionally the desire to get things done, in work load that required concentration, besides the effort and duration in the accomplishment will burden the sensory into rejection, out of fear in the cumbersome of day in and day out routine. Upbringings in the early stages, especially the intimidations, somehow have effect on a high-strung emotion that ramifies into severity of personality changes. In this case: More than meets the eye, in reference to the causality of fear in everyday lives.

My personal opinion is that there is a spiritual void (a big empty hole in themselves) that people don't know how to fill or what to fill it with. The fear comes from not being complete with themselves. Who am I, why am I here, what's my purpose...
People will try to fill that hole with a lot of different things to feel right with themselves. I have and only found one that worked for me.

Root cause? Focus on "self" rather than something greater. I have no hope in myself, my hope is in "that than which nothing greater can be concieved" (to quote Anselm). Since He (God) is greater, than He can conquer my fears, and give me hope.
When I trust Him, rather than focusing on self or on fears I have, than I am not so blinded to the truth. Fear does blind us.

We fear because of our ignorance. Examples:
1. Fear of death is caused by our lack of knowledge of what happens after we die; if we knew for sure, we wouldn't be so afraid
2. Fear of flying is because we are afraid to die, so back to first point.
3. Fear of heights is because we are afraid we will fall... to our death... back to point #1
... and so on.
