In terms of video games, they don't have their names displayed because they don't have contracts with the game makers. NFL games can show the names of players that are in the Players' Association because they are under contract. People like Parcells were never in the NFLPA, that's why his name didn't appear in the video games (that's also why some rookies in NFL video games don't have names, they aren't signed with the NFLPA yet.)
Unless the makers of the game got a contract with every player at every school, they won't put their names in.

In the video games they cannot have the names on because it is against NCAA policy to have the real names of the players shown.

NCAA rules prevent promoting players. So the games aren't allowed to show the name as it would be a violation of the rules. The NCAA claims it is about keeping the players armature.

Depends on the school, I'd say the majority of NCAA teams do have their name on their jersey. (I think that's what you were asking?)

Because they are amateurs...not professionals...and actually, its the schools'/coaches' decision...Some schools do have names...other schools with class, like Notre Dame, don't use names...
