I had it fixed. The service man after so many tries figured it out, he was lazy and kept telling me don't put heavy load even though I was putting light load. These models don't come with nut fully tightened at the bottom from factory. This makes the belt wobble and come off. Like Alex mentioned if belt is pulled put a new belt and tighten the nut properly, you should be able to do this yourself by looking under the machine.

take the top off. loosen nuts on motor with the belt on pull up if the motor wont pull no more and the belt is still slack then get a new belt. put the new belt on pull the motor till belt is tight hold while you tighten nuts back up should work fine

If your machine is old..Best to cut your losses and get a new one.
Machines are quite cheap these days.

it could be the barrings like somebody said but they would have to be really badly gone to put the fly wheel on tilt. to check barrings have gone look below centre of wheel on the plastic drum, you brown mark on it. also the m/c will be making one hell of a noise when you put on spin cycle.

your tension adjuster seems to be missing, or your belt has stretched, try a new belt first
