If I should Sex of Child or Gender of Child here?

It should be "gender."

Or to put it another way, “Male” and “Female” are sex categories, while “masculine” and “feminine” are gender categories.

You're not exactly clear on what the form states...
in that particular box or line is it asking you for the "Sex" or the "Gender"?
It sounds like you are simply volunteering the information.

Forms that I'm aware of however ask for "Gender" when wanting to know whether male or female.

Sex is correct. Gender is a much abused euphemism.

Sex refers to male or female and gender refers to which sex he/she most relates or associates with
