I'm not sure why you're asking the this section, but how about something like:

"To Whom It May Concern,

I wanted to write to let you know that Jill Smith provided me with the highest level of service recently.

Specifically, on xx/xx/xx, I called/was in your store/etc. and a problem presented itself. Ms. Smith took it upon herself to do such-and-such, which was unexpected, and was greatly appreciated.

Please know that your customers value this type of service. I will be sure to let my friends and associates know.

Thank you for creating the kind of environment where someone like Ms. Smith works and wants to go above and beyond for the customer."

Too long, perhaps, but you get the point...
