A card.

A meaningfull self written card, not something made in Microsoft Publisher, to to Hallmark and buy something nice. Although cards may not seem like much to the writer, they hold unending meaning for the person on the recieving end.

A poem would also do nicely. Depending on the nature of the gifting.
