[name of individual] contributions helped to move the [project] forward which made it possible to meet the projected deadline. [name] assistance was extremely valuable and I appreciate the extra time [she/he] gave towards the completion of the [project].

You want to write a letter to the supervisor, a letter to whoever is above the supervisor and the person who helped you (which is optional). Indicate in the letter who you are sending it to, the date the action occurred, what the action was, why the action was above and beyond the call of duty, how you felt before and after the action, and what you would like to come of the letter(s).

EXAMPLE: Firefighter
Letter to the Captain of the company, Chief of the Department, Mayor and Council (or ruling body of the municipality) and your choice if you want to send it to the firefighter or not.
