Contact social security office and show them your ss card with your married name on it, tell them you are divorced and want to change your name back to your maiden name. You will have to show some other proof of identity. Then go get your license changed and just start using your maiden name on all your correspondence.

You can ask for it in your divorce decree. Otherwise, if you have not asked for it, it's possible to petition the court for a name change like anyone else would do. It is usually easiest to go back to your maiden name during the process of divorce. Some people also just start using their maiden name again, by changing their drivers license, social security card, etc. this is somewhat easy since chances are your birth certificate is still in the maiden name and you will only need a copy of your marriage license to show why it was changed in the first place. If you are getting divorced, put that in the Petition for divorce that you want to go back.
If you are already divorced but didn't change your name at the time OR you are married but you want to change your name back, you need to file some paperwork for a name change with the court - call the court and ask them what you need to do, they should be able to give you some forms. If they can't, get a family law attorney to help you. It shouldn't cost much.

Nowadays you don't have to have it put in your divorce papers to go back to your maiden name, it is in there automatically. The next step is for you to do ALL of the work getting it changed, you have to change it on your Socicial Security card, driver's license, any credit cards, insurance papers, medical/dental records, any loans, if you are renting a place you will have to have it changed on your lease, if you are employed you will have to go to your H.R department and have all of your paperwork changed, and you will have to change all of your tax information. I suggest you wait until AFTER you file this year's taxes and get the returns back...

If you are going thru a divorce right now, put that in your divorse papers. I did and I got my maiden name back. If you are already divorced you still have to contact the court to change it and will probably be a fee to do it.

go to your local court house and talk with them and see what advice they have on this. You may need a lawyer to help you as well but i am not sure.
