I would get them at abercrombie/ Abercrombie & Fitch, but they're kind of expensive (40-100 dollars...or more). Hollister and American Eagle are a little bit cheaper and are just as nice.
If you have time to do it yourself, use sandpaper to make your jeans look worn out. Use scissors to cut small hole then rip it as big as you want by yourself (As in, without the scissors).

do it yourself
just cut with scissors
