Funny you should ask. About an hour ago I was giving a snake program (a Q&A session with park visitors) with one of our corn snakes, but I didn't realize that she wasn't "walked" before hand. Well anyway, right in the middle of my program she lifted her tail and ishhed all over the auditorium carpet in front of everybody. It caught me off guard so bad that I bombed the rest of the way through my program.
I ran out to grab paper towels to cover it up with and while I was out there, I told my co-worker what happened, but I didn't realize the microphone loud speaker was on, so it got broadcasted outside.
This isn't fire-worthy... just a bad sequence of events. I'm sure I'll laugh at it when I return to work tomorrow.

I transposed a part number on a material order for a project. The part was special order and could not be sent back. Cost the project $15K, but I didn't get fired. Just got a royal chewing out. Haven't ordered the wrong part now for years.

I allowed a customer to leave the store without paying for the product, even though he left me his information. The result was that it was bogus information and I got stuck with nearly $50 worth of gas not paid. I didn't get fired because i was the store owner. But it still sucked.

Other than accidentally not showing up, I handed out three wrong orders in an hour with the GM's boss there. He was not happy with me but I didn't get fired.

I entered a stolen tag incorrectly (transposed the letters) and these people got stopped by the police in West Virginia on a felony stop. We could have been sued, but they didn't...and I didn't get fired.
