One of the most obvious would be the computer. Lost production due to personal emails, web browsing, IM'ing and various games that have nothing to do with the work product (for instance asking and answering questions on Yahoo answers would be a good example if done from work)

lack of skills will affect productivity - if you are not well-skilled for the job. The other factor is managerial issues - leading to lack of motivation. If people dont understand how the fit in to the whole organisation, they will lack motivation.
Unsatisfatory (low) Income is probably the most important factor.
