No quotes but do center and all caps please. I've included some websites that shows exactly how an essay should be set up. My favorite one is the this one.

Honestly i have never heard of either of those things being required i suppose it would depend on your teacher. If it is a title of a published book underline it if it is your own ork probablly underline it as well

If you are in highschool, and it's a formal essay, you do neither. It may look boring, but that is the proper way to do it, unless your teacher told you to do something else. If the title is the name of a book, then you can italicize, but if it is your own title, you leave it alone.

for the title page? you can underline it, or just leave it how it is. if you are mentioning the title in your essay, you have to italisize the title.
