The jews were made to be scapegoats for the problems of Germany in the 30's such as economic depression. The Nazi's believed they were doing their country a service by getting rid of the jews. There was enough discontent because of the great depression and the post-world war I treaties that this was possible.
They were in many cases gathered together in "ghettos", sectioned off areas of existing cities such as the Warsaw ghetto. Later on the "ghettos" were used as feeders for the concentration camps as the jews were told that they were being "relocated", most often by train.

Claude Lanzmann made a documentary called Holocaust. It's the most detailled document ever made. He talked to hundreds of survivers from the holocaust and revealed in an extraoridinary way how it could happen.
How? Make the victims do the dirty job themselves.

They did it because they wanted to kill all the Jews. Most went by train, packed in like cattle. Many died on the way. They were the lucky ones.
