There have been many. The Japanese attack on Pearl Harbor comes to mind. They seriously underestimated the strength and resolve of the United States and got nuked as a result.
But there have been national mistakes that make that one pale by comparison. The German people made a big one when they got behind Adolf Hitler, and I suppose the Russian experiment with Communism was a major error too since in both cases, millions of people were slaughtered.
We should learn from their mistakes and vow to never let a tyrant take over our country, or at least have the intelligence to recognize one before it's too late.

-Global Warming/Neglecting Earth's health.-
For some time now, we've had a chance to do something about it, and a lot of people blew it off. Now that every one is starting to realize, that the Earth could get hotter than ****, it might be too late. Some don't even care. I've, more than once, heard people say, " Oh I don't need to worry about that, I'll be dead by the time it comes around." Some people are so lazy and ignorant. I believe that is a bigger mistake than any one out there. Ignorance.

Looking at our differences, not our similarities.
People get hung up on what nationality people are, what race, what gender, what religion, what occupation, etc. But when you take all that away and just look at people as people, we're not all that different. Its really easy to hate someone who's different from you, but a lot harder to hate someone once you realize how much you are the same. I think if the world focused more on our similarities, there would be less violence, less poverty, probably less diseases, too, as scientists from around the world could come together to work on cures.

Self obsession, never ending need for money, power, etc...
The feeling of inadequacy no matter however much we get, one is never satisfied, comparing to others endlessly, herd mindset for acceptance and no prominence for self concept of life...
To coin it in a line- It all roots un in one's mind, the greatest problems of the world!
