you shouldn't have to as long as the new door was made to suit the original opening. if not, your builder can fix it all. but like everything in life, it'll cost you. ask your door supplier. best of luck.

If your arch has a key stone in the middle,then no you don't, just fit the frame and block the gap above it with either cladding or bricks,and that will be fine

not if the lintol is in good condition ..and you are not making the opening any wider
