In the middle of a bitter custody dispute and my child's counselor is advising my child's other parent to act against court orders. We have joint legal custody so all medical decisions are made by both parents. I signed a consent form for her to treat my child when we signed him up, but now that I know she is not professional, how do I revoke my consent?

write a letter saying specifically that you revoke your consent and what you gave consent for, say also that you feel it is in the best interest of your child that you do this. don't forget to sign it, date it and make a few copies of it. deliver it to the person that you are having trouble with. if they have any problems understanding your revocation then seek their supervisor and let them know- give them a copy of the letter as well if they want one- what ever you do always keep a copy for yourself.

You start by drafting and titleing the paper as such:

NOTICE OF INTENT TO REVOKE


I, (your name), being the parent of (childs name) do hereby
Revoke, Recend all authorizations given by my hand upon enrollment of the herein named child into (schools name). I futher attest under penalty of perjury and prosecution that I am authorized under Parents Custodial Rights to Revoke,and/or Recend any and all previous authorizations given by my hand in signature and/or verbal form. This Notice shall take effect immeadiately and be binding on the (schools name) administration and the medical personell.

Go to the counselor and ask to have it revoked. If they do not get a letter from an Attorney.

File for the revocation of the consent by citing valid reasons.
